ABSTRACT

Background. Scabies is an ectoparasitic, highly contagious skin disease caused by a human itch mite infes tation of the skin, and it is the leading cause of morbidity and disease burden in developing countries. The purpose of this study was to investigate healthcare users' current knowledge and experiences with scabies management provided at primary healthcare facilities. Materials and Methods. A qualitative research design was used to address the research objectives. Focus group interviews with 58 health care users were used to collect data. Thematic analysis was performed on the verbatim transcriptions using Tesch's approach. Results. The findings showed that the knowledge of healthcare users who participated in the study on scabies, its management, prevention, and control was limited. Health care users experience different chal lenges regarding scabies and the management thereof. Five themes emerged after data analysis. These were knowledge regarding scabies, knowledge regarding the management of scabies, knowledge regarding the prevention of scabies, perceptions regarding receiving treatment for scabies, and recommendations regarding the availability of materials and medication. Conclusions. Healthcare users in the area had limited knowledge and experienced different challenges regarding scabies and their management. These challenges contribute to low quality health services with undesir able health outcomes. To narrow this gap, consistent and programed health education was provided to the community through different modalities by using the existing health system to increase awareness regarding scabies. Following the implementation of community awareness, each commu nity member was positioned to prevent and control scabies.

ABSTRACT

Scabies is an ectoparasitic infection caused by Sarcoptes scabiei var. hominis in humans. Accurate figures of the prevalence of scabies are difficult to obtain, it seems to affect around 100�0 million people worldwide. Scabies is usually transmitted by close intimate physical contacts, such as the sharing of a bed or prolonged hand-holding. Scabies mites survive for 24� h outside of human bodies. Itching is the most obvious manifestation of scabies, which is usually sparing the face in adult classic scabies. The onset occurs 3�weeks after acquiring the infection. Bullous pemphigoid-like scabies is one of the atypical and rare forms of scabies that may be overlooked and mismanaged, where only 44 cases worldwide with this mode of presentation were reported. Here, we reported a series of three cases of bullous pemphigoid-like scabies, which presented with nocturnal itching and vesiculobullous lesions that are to the best of our knowledge the first cases reported in our country.

ABSTRACT

Crusted scabies is an uncommon manifestation of parasitic infection caused by Sarcoptes scabiei var. hominis. This variant of scabies is seen in various immunocompromised states. Uncontrolled diabetes is an immunocompromised state which is characterized by increased incidence of various infections, and rarely, may be associated with this rare crusted scabies, which may pose a real diagnostic challenge. A high index of suspicion is required for timely and correct diagnosis in the best interest of the patient and the public health point of view to prevent the spread of this highly contagious infestation.

ABSTRACT

En esta revisión de la literatura se describen aspectos epidemiológicos, fisiopatológicos, clínicos y terapéuticos sobre una presentación atípica y grave de escabiosis, la sarna costrosa o noruega. Esta presentación de escabiosis destaca por afectar principalmente a personas con condiciones de inmunodepresión o sociales que las hacen susceptibles de una alta carga parasitaria, además se asocia a un peor pronóstico y a riesgo de complicaciones. Desde el punto de vista terapéutico, sus estrategias difieren del manejo de la escabiosis clásica.(AU)


This literature review describes epidemiological, pathophysiological, clinical and therapeutic aspects of an atypical and severe presentation of scabies, Norwegian or crusty scabies. This presentation of scabies stands out because it mainly affects people with immunosuppressive or social conditions that make them susceptibleto a high parasite load, it is also associated with a worse prognosis and risk of complications. From a therapeutic point of view, their strategies differ from the management of classic scabies.(AU)

INTRODUCCIÓN: la escabiosis constituye un problema de salud pública en especial en los niños, es una infestación cutánea, altamente pruriginosa y contagiosa endémica en países en vías de desarrollo, afecta a todos los grupos socioeconómicos y a todas las edades es causa la alta morbilidad OBJETIVO: determinar la información sobre prevención de escabiosis que poseen los representantes de preescolares de 5-6 años de la comunidad de la Peña posterior a una intervención educativa MATERIALES Y MÉTODOS: investigación cuasi experimental de pre test y post test de las intervenciones educativas. El universo estuvo conformado por 143 representantes de preescolares, la muestra intencionada fue de 86 personas, el instrumento fue un cuestionario conformado por ítems de respuestas dicotómicas que se administró a los representantes antes de la intervención educativa y posterior a la misma. RESULTADOS: antes de la intervención existía muy baja información acerca de la prevención de la escabiosis lo que representaba el 67.4 %. Posterior a la misma se incrementa el número de representantes que mejoran su nivel de información hasta llegar a 73.2%. CONCLUSIÓN: El desarrollo y aplicación de la intervención educativa logro elevar los conocimientos sobre que es la escabiosis identificar cuáles son los factores de riesgo.


INTRODUCTION: scabies is a public health problem, especially in children. It is a highly pruritic and contagious skin infestation endemic in developing countries. It affects all socioeconomic groups and causes high morbidity at all ages. OBJECTIVE: to determine the information on the prevention of scabies that the representatives of preschool children aged 5-6 years from the Peña community have after an educational intervention. MATERIALS AND METHODS: quasiexperimental pre-test and post-test research of educational interventions. The universe was made up of 143 preschool representatives, the intended sample was 86 people, and the instrument was a questionnaire made up of items of dichotomous responses that were administered to the representatives before and after the educational intervention. RESULTS: before the intervention, there was very little information about the prevention of scabies, which represented 67.4%. After the same, the number of representatives that improve their level of information increases to 73.2%. CONCLUSION: the development and application of the educational intervention managed to raise the knowledge about what scabies is, to identify which are the risk factors.


INTRODUÇÃO: a escabiose constitui um problema de saúde pública principalmente em crianças, é uma infestação cutânea altamente pruriginosa e contagiosa, endêmica em países em desenvolvimento, afeta todos os grupos socioeconômicos e é causa de alta morbidade em todas as idades OBJETIVO: determinar as informações sobre prevenção de sarna que os representantes de crianças pré-escolares de 5-6 anos da comunidade La Peña têm após uma intervenção educativa MATERIAIS E MÉTODOS: pesquisa quase experimental de pré-teste e pós-teste de intervenções educativas. O universo foi constituído por 143 representantes da pré-escola, a amostra pretendida foi de 86 pessoas, o instrumento foi um questionário composto por itens de resposta dicotómicos que foi aplicado aos representantes antes e após a intervenção educativa. RESULTADOS: antes da intervenção havia muito pouca informação sobre a prevenção da escabiose, o que representava 67,4%. Depois disso, o número de representantes que melhoraram seu nível de informação aumentou para 73,2%. CONCLUSÃO: o desenvolvimento e aplicação da intervenção educativa foi capaz de aumentar o conhecimento sobre o que é sarna para identificar quais são os fatores de risco.

ABSTRACT

Abstract Scabies is an ectoparasitosis caused by Sarcoptes scabiei var. hominis, characterized clinically by pruritic lesions in typical locations; the crusted form is a rare manifestation. The diagnosis is usually established based on the clinical picture, but dermoscopy can be an important complementary method, as it allows the observation of a brownish triangular structure with a hang-glider appearance. A case of crusted scabies is reported; the magnification of the images obtained by digital dermoscopy allowed the demonstration of a structure usually observed only with videodermoscopy.

ABSTRACT

This study aimed to analyze the parasites infesting pigs and their producers on family farms in Tanguá, RJ, Brazil, and to conduct extension activities between 2018 and 2019. Fecal samples were collected from 132 pigs, as well as, skin scrapings from the ears of 125 animals. In addition, 36 stool specimens from farmers and their families were analyzed. The collected material was processed by direct examination, sedimentation and flotation techniques. Gastrointestinal parasites were detected in 88.6% of the pigs, especially coccidia (71.8%), Balantioides coli (55%), strongyles (40.2%) and Strongyloides ransomi (31.6%) which showed statistically significance (p<0.05). Ectoparasites were identified in 16% of the pigs, mostly Sarcoptes scabiei var. suis (76.2%). Parasites were detected in 19.4% of the humans, including, Entamoeba coli (19.4%), Ascaris lumbricoides (8.3%) and Trichuris trichiura (5.5%). The frequency of B. coli and S. scabiei var. suis (p<0.05) were statistically significant, when analyzing the age group of the animals, having been more detected in the older years, fatterners phase. The pig farmers participated in these activities extension, which included lecture and "field day", aimed at imparting information about parasites and their prophylaxis. The situation in Tanguá pig farms reinforces the need to carry out programs that provide technical support to these farmers, whose livelihood depends on pig farming.(AU)


Este estudo objetivou analisar os parasitos que infectam suínos e seus produtores em propriedades familiares em Tanguá, RJ, Brasil, realizando atividades extensionistas entre 2018 e 2019. Amostras fecais foram coletadas de 132 suínos, bem como raspados da pele da orelha de 125 animais. Além disso, 36 amostras fecais dos produtores e seus familiares foram analisadas. O material coletado foi processado pelo exame direto e por técnicas de sedimentação e flutuação. Parasitos gastrointestinais foram detectados em 88,6% dos suínos, principalmente coccídios (71,8%), Balantioides coli (55%), estrôngilos (40,2%) e Strongyloides ransomi (31,6%) que apresentaram significância estatística (p<0,05). Ectoparasitos foram identificados em 16% dos suínos, principalmente Sarcoptes scabiei var. suis (76,2%). Parasitos foram detectados em 19,4% das amostras fecais humanas, incluindo Entamoeba coli (19,4%), Ascaris lumbricoides (8,3%) e Trichuris trichiura (5,5%). A frequência de B. coli e S. scabiei var. suis foi estatisticamente significativa (p<0,05), quando se analisou os animais por faixa etária, tendo sido principalmente detectado nos animais mais velhos, como na fase de terminação. Os produtores de suínos participaram ativamente das atividades de extensão, incluído a palestra e a atividade "dia de campo", que tinham como objetivo mediar informações sobre parasitos e suas medidas profiláticas. A situação nas granjas familiares de Tanguá demonstra a necessidade de realização de programas que apoiem tecnicamente esses produtores, que utilizam a suinocultura como fonte de subsistência.(AU)

ABSTRACT

Background: The objective of the study was to find out the personal hygiene status among boys residing in social welfare hostels by class and age and type of hostel.Methods: Cross sectional study, analytical study, social welfare hostels in Tirupati town of Andhra Pradesh. Participants was 524 boys. Statistical analysis are percentages and Chi-square test.Results: The prevalence of scabies was found to be significantly higher in those children with poor personal hygiene (35.7%) compared to average level (18.8%) and good (14.4%) of personal hygiene. The prevalence of other specific morbid conditions was found to be similar in all the three levels of personal hygiene and the differences were not statistically significant.Conclusions: The prevalence of scabies was found to be significantly higher in those children with poor personal hygiene compared to average level and good of personal hygiene. In view of higher proportion of children with poor level of personal hygiene of children in scheduled tribe hostel, there is a need to sensitize and give health education on personal hygiene related diseases and take necessary preventive steps and supervision by hostel staff especially in scheduled tribe hostel. Hand washing with soap can protect about one out of every three young children.

ABSTRACT

Paciente do sexo feminino, com 59 anos de idade, portadora de mastocitose sistêmica há 20 anos. A mastocitose é doença rara, caracterizada pela proliferação excessiva e o subsequente acúmulo de mastócitos em órgãos e tecidos, principalmente na medula óssea, pele e no trato gastrointestinal. Há 1 mês, relatava história de novas lesões cutâneas caracterizadas por pápulas e placas eritemato-edematosas com escoriação e intenso prurido. Feito o raspado da pele com confirmação diagnóstica de escabiose.


A 59-year-old female patient had a diagnosis of systemic mastocytosis for 20 years. Mastocytosis is a rare disease characterized by excessive proliferation and accumulation of mast cells in organs and tissues, especially in the bone marrow, skin and gastrointestinal tract. She reported new skin lesions characterized by erythematous papules and plaques with excoriation and intense itching for one month. Skin scraping confirmed the diagnosis of scabies.

ABSTRACT

Background: Scabies awareness can decrease the disease health burden. The objective was to assess the awareness and knowledge among people with scabies in a questionnaire based cross sectional, observational study.Methods: Study was questionnaire based, cross sectional, descriptive study. 200 consecutive patients with scabies were enrolled for it.Results: Of the 200 participants, 125 (62.5%) were males and 75 (37.5%) were females. People of all educational levels were affected from scabies ranging from illiterate 25 (12.5%) to graduates 44 (22%). 95 (47.5%) participants did not knew that itching can be because of scabies. 171 (85.5%) participants had no idea of mode of spread of the disease. 199 (99.5%) participants had no knowledge of treatment. None of the participants knew that scabies is preventable.Conclusions: There is a general lack of awareness and knowledge among the people regarding various aspects of scabies. Increased health awareness programmes with more educational and psychosocial support should be provided to the people. Mass drug treatment of scabies in the area should be provided instead of treating the individual patients

RESUMEN Introducción: la llamada «Escabiosis o Sarna¼ es una infestación producida por el acaro Sarcoptes scabiei, caracterizada por lesiones generalizadas pruriginosas. Objetivo: describir su comportamiento a través de las variables sociodemográficas edad y sexo; área de salud, condiciones socioeconómicas, mes de aparición; localización de las lesiones, y principales signos y síntomas que motivaron la consulta. Método: se realizó un estudio descriptivo, longitudinal, ambipectivo, a niños ingresados en el servicio de miscelánea con el diagnóstico de escabiosis en el periodo de agosto de 2018 a febrero de 2019, El universo estuvo constituido por 240 casos con piodermitis y la muestra fueron 192 casos con escabiosis. Se utilizaron estadígrafos descriptivos (frecuencias absolutas y relativas), los datos se procesaron a través del programa profesional estadístico SPSS 22.0 para Windows. Resultados: el 69.7% correspondían al grupo de edad menores de 1 año, del sexo masculino el 52,6%, el área de salud Policlínico Rene Vallejo con 27,3%, el 45,8% de las condiciones socioeconómicas fueron malas, el 35,4% de las lesiones fueron en el cuero cabelludo, el 100% manifestó prurito, erupciones mucocutáneas típicas y malestar general, el mes que predomino fue el mes de enero con 115 pacientes para un 59,8%. Conclusiones: predominó el grupo de edad menor de 1 año, el sexo masculino, el policlínico René Vallejo aportó el mayor número de los casos, las condiciones socioeconómicas fueron malas, las manifestaciones que predominaron fueron el prurito, la erupción mucocutánea y el malestar general y el mes de aparición fue en enero.


ABSTRACT Introduction: the so-called «Escabiosis or Scabies¼ is an infestation produced by the Sarcoptes scabiei mite, characterized by widespread pruritic lesions. Objective: to describe their behavior through the sociodemographic variables age and sex; health area, socioeconomic conditions, month of appearance; location of the lesions, and main signs and symptoms that motivated the consultation. Method: a descriptive, longitudinal, ambipective study was conducted on children admitted to the miscellaneous service with the diagnosis of scabiosis in the period from August 2018 to February 2019, The universe consisted of 240 cases with pyodermitis and the sample were 192 cases with scabiosis. Descriptive statisticians (absolute and relative frequencies) were used, the data was processed through the statistical professional program SPSS 22.0 for Windows. Results: 69.7% corresponded to the age group under 1 year old, 52.6% male, the Rene Vallejo Polyclinic health area with 27.3%, 45.8% of the socioeconomic conditions were bad, the 35.4% of the lesions were in the scalp, 100% manifested pruritus, typical mucocutaneous eruptions and malaise, the month that prevailed was the month of January with 115 patients for 59.8%. Conclusions: the age group under 1 year predominated, the male sex, the polyclinic René Vallejo contributed the largest number of cases, the socioeconomic conditions were bad, the predominant manifestations were pruritus, mucocutaneous eruption and general malaise. The month of appearance was in January.

ABSTRACT

Se expone el caso de una paciente obesa inmunodeprimida que presentó una sarna costrosa. Luego de la sospecha clínica se confirmó el diagnóstico mediante acarotest. La paciente sufrió algunas complicaciones asociadas a su condición general, como sobreinfección de sus lesiones cutáneas, epistaxis e insuficiencia renal aguda, que fueron tratadas. La sarna costrosa fue tratada con ivermectina oral con dosis de 15 mg (200 ug/kilo de peso ideal según la talla), los días 1, 2, 7, 8 y 15, obteniendo una excelente respuesta terapéutica.


We present the case of an immunosuppressed obese patient who presented with crusted scabies. After clinical suspicion, the diagnosis was confirmed with skin scraping for the diagnosis of scabies. The patient presented some complications associated with her baseline condition, such as superinfection of her skin lesions, epistaxis and acute renal failure, which were treated. Crusted scabies was treated with oral ivermectin with a dose of 15 mg (200 ug/kg of ideal weight according to height), on days 1,2,7,8 and 15, obtaining an excellent therapeutic response.

@#INTRODUCTION: Norwegian or crusted scabies is a rare and highly contagious form of skin parasitosis caused by Sarcoptes scabiei var. hominis. Individuals maffffinly affected are considered to be immunocompromised such as those on prolonged glucocorticosteroid therapy, with AIDS or organ transplant patients. This disease presents as a hyperkeratotic dermatosis with an acral distribution. CASE REPORT: This is a case of a 2-month-old healthy Filipino male, who was previously managed as a case of miliaria rubra and treated with clobetasol 0.05% – ketoconazole 2% cream for 1 week. The papules and plaques became widespread. Consult with a pediatrician revealed widespread scabies and for which patient was prescribed topical permethrin with no improvement. On examination, patient presented with multiple erythematous papules and plaques with crusts on the face, trunk, extremities, palms and soles. Thickened yellowish plaques were observed on the palms and soles. Both parents also presented with widespread papules most prominent on the flexural areas accompanied by nocturnal pruritus. On dermoscopy, numerous mites and burrows were seen in a “jet with contrail pattern.” Prominent yellowish scales were also noted. Patient was admitted due to fever and superimposed bacterial infection and was given IV oxacillin, paracetamol, 8% precipitated sulfur in a hypoallergenic lotion applied twice daily and sodium fusidate ointment. On the 4th hospital day, the patient was afebrile and the lesions were noted to decrease in both erythema and crusting. Follow-up dermoscopy revealed absence scales, burrows and mites. CONCLUSION: Prolonged, unsupervised use of topical corticosteroids in our case most likely induced an immunocompromised state thus predisposing the patient to develop Norwegian scabies. In countries were cases of Norwegian scabies have been unresponsive to permethrin and when ivermectin is not available, the use of precipitated sulfur may still be the best therapeutic and safest option for infants.

Background: Efficacy of these modalities as shown by various investigations are inconsistent and ambiguous. Thus, evidence based effective treatment option is warranted. Aim of the study was to compare the efficacy of oral ivermectin, topical permethrin and benzyl benzoate in the treatment of uncomplicated scabies.Methods: Patients with confirmed diagnosis of scabies were included in this study. One hundred and ninety-five subjects were included in this investigation as per inclusion and exclusion criteria laid down. Equal numbers of patients were randomly allocated to one of the three treatment groups. Efficacy of three groups [oral ivermectin (Group A), topical permethrin (Group B) and benzyl benzoate (Group C)] of drugs was compared in terms of improvement in clinical grading of disease (%) and improvement in clinical grading of pruritus (%) during follow up visits.Results: Those subjects receiving topical permethrin, at 1st follow up 56.9% showed cure rate which increased to 89.2% at 2nd follow up with respect to clinical improvement in pruritus. Maximum relief in severity of pruritus at the end of 6th week was reported by 58(89.2%) patients receiving group B treatment modality followed by 52 patients (80%) in arm A. Regarding efficacy of three treatment groups in terms of improvement in severity of lesion at the end of 6 weeks, maximum number of patients 57(87.7%), receiving group B treatment reported improvement which is better than other two treatment groups.Conclusions: maximum number of patients receiving topical Permethrin treatment reported improvement better than other Oral Ivermectin therapy and topical benzyl benzoate. Oral ivermectin may serve a good alternative for managing scabies under certain conditions like poor compliance to topical scabicides.

ABSTRACT

La sarna costrosa corresponde a una variedad muy rara y poco frecuente de escabiosis; una parasitosis humana causada por el ácaro Sarcoptes Scabiei (var. Hominis); se produce principalmente en pacientes inmunodreprimidos y se caracteriza por lesiones atípicas y extensas, sumamente contagiosas debido al compromiso inmunitario. Se presenta el caso clínico de un paciente de 68 años de edad con antecedentes de hipertensión arterial y diabetes mellitus tipo 2 con un mal control glucémico, que es atendido por el servicio de dermatología. Las lesiones se presentan como placas extensas pruriginosas, queratósicas, eritemato-escamosas de escamas gruesas y adherentes de seis meses de evolución en tronco, muslos y pliegues axilares, inguinales e interdigitales. La sospecha clínica de sarna costrosa fue confirmada por biopsia; el tratamiento se realizó con ivermectina con mejoría del cuadro clínico.


Crusted scabies corresponds to a very rare variety and little common scabies; a human parasitic disease caused by an acarid in this case Sarcoptes Scabiei (var. Hominis); it observed mainly among inmunosupressed patients and is characterized by atypical and extensive lesions, highly contagious due to their immune compromise. A case of a patient of 68 years old is described with a history of hypertension and diabetes mellitus type 2 with poor glycemic control, which is assist by the dermatology service. Lesions included extensive and pruritic badges, quetatotic, erythematous-squamous of thick and adherent scales of six months evolution in trunk, thighs and axillary, inguinal and interdigital folds. The clinical suspicion of crusted scabies was confirmed by biopsy; the treatment was performed with ivermectin with improvement of the clinical picture.

Resumen Introducción: La escabiasis costrosa (EC) es una variante poco común de sarcoptiosis clásica, altamente contagiosa. Las lesiones poseen una elevada concentración del ácaro Sarcoptes scabiei var hominis, lo que conlleva a un cuadro clínico más extenso que en la escabiasis clásica. Se observa principalmente en pacientes con algún tipo de inmunocompromiso y se relaciona con el síndrome de Down. Caso clínico: Se describe una paciente pediátrica con síndrome de Down quien presentó placas escamosas que afectaron la porción distal de los dedos, asociadas con distrofia ungueal e hiperqueratosis subungueal, por lo que se consideró acrodermatitis continua de Hallopeau como diagnóstico diferencial. Se realizó una biopsia tipo punch con lo que se llegó al diagnóstico de EC. La paciente recibió tratamiento sistémico con ivermectina vía oral y tratamiento tópico con crema hidratante y desonida al 0.1%. Mostró mejoría clínica notoria dos semanas después de finalizar el tratamiento. Conclusiones: La EC es una variante prevalente en pacientes inmunocomprometidos y con síndrome de Down que fácilmente puede confundirse con patologías inflamatorias con alteración de la queratinización epidérmica. Este caso se considera una presentación atípica debido a la afección localizada en los dedos de las manos asociada con distrofia ungueal. El estudio histopatológico fue necesario para realizar el diagnóstico y descartar diagnósticos diferenciales.


Abstract Background: Crusted scabies (CS) is an uncommon, highly contagious, variant of classic scabies. Elevated concentrations of the mite Sarcoptes scabiei var. hominis are found in the skin lesions, which lead to a more exaggerated clinical picture than in classic scabies. This disease is mainly observed in patients with any kind of immunosuppression and relates to Down syndrome. Case report: A pediatric female patient with Down syndrome, who presented a crusty white plaque associated with nail dystrophy and subungual hyperkeratosis affecting the distal portion of the fingers is described. Because of these findings, the diagnosis of acrodermatitis continua of Hallopeau was considered. A punch biopsy was performed, attaining the diagnosis of CS. She received systemic treatment with oral ivermectin, topical treatment with emollient cream and desonide 0.1%. Notorious clinical improvement was observed two weeks after finalizing treatment. Conclusions: CS is variant of scabies prevalent in immunocompromised patients and Down syndrome that can be easily confused with inflammatory pathologies with abnormal epidermal keratinization. This case is considered as an atypical presentation of the disease because of local affection of the fingers and nail dystrophy. The histopathological study was necessary to obtain the diagnosis and rule out differential diagnosis.

ABSTRACT

Introduction: Scabies is a ectoparasitic infestation casued by the mite Sarcoptes scabiei. Its mainifestation includes itching,excoriations, papular lesions and burrows. It is highly contagious, affecting usually all the members of a family staying togetheror inmates of a hostel. It is convenient to treat the affected ones but treating all the contacts is difficult, resulting in reinfection.Aims and Objectives: In this study we try to evaluate if application of topical scabicidal creams such as permentrin 5% iseffective in curing the disease.Materials and Methods: A total of 56 patients who presented to our Out patient department were included for the study. Theywere prescribed permetrin 5 % cream. Advice regarding the proper application of the cream and treatment of contacts wasgive. They were followed up at the OPD at 2 weeks, 1 and 2 months for assessment of response.Results: Our study showed that although effective initially in a significant number of patients, there was a considerable relapseof the infestation. Only 11 patients (19.64%) showed complete response with failure in the rest (80.36%).Conclusion: Although scabies is a common parasitic infestation, its treatment is complex. The difficulty in treating scabies isidentification and treatment of all of the contacts, many of whom are asymptomatic. The average cost of permetrin 5% cream isaround Rs 90, the cost involved in treating all the contacts such as family members and inmates in hostels would be far morethan what the common man can afford. This makes the treatment of scabies unfeasible especially in developing countries likeIndia. Hence scabies which is a self limiting infestation, is best treated symptomatically

BACKGROUND@#Scabies is a skin infestation caused by the mite Sarcoptes scabiei that causes a pruritic skin eruption. In Ethiopia, the disease is common especially during natural or manmade disasters such as flooding, drought, civil war and conflict, poor water supply and sanitation, and overcrowding living condition. Though scabies is becoming a major public health problem, especially in low resource settings, there has been no study that reported the prevalence of scabies in the study area. The aim of this study, therefore, was to assess the prevalence of scabies and associated factors among students in primary schools in Dabat district, northwest Ethiopia.@*METHOD@#An institutional-based cross-sectional study was employed to determine the prevalence of scabies and associated factors among schoolchildren in Dabat district. A total of 494 students selected by a multi-stage sampling technique were included in this study. Scabies was diagnosed by medical practitioners based on lesions observed on body surfaces after a physical examination. Bivariable and multivariable binary logistic regression analyses were performed using SPSS version 20. Significance level was obtained at p value < 0.05.@*RESULT@#The prevalence of scabies was 9.3% (46/494) with 95% CI (5.66%, 12.94%). Among schoolchildren who were infested by scabies, 65.22% (30/46) had a mild, 28.26% (13/46) had moderate, and 6.52% (3/46) had severe lesions. Studying at a rural school (AOR = 2.99, 95% CI 1.33, 6.71), had illiterate father (AOR = 5.11, 95% CI 2.25, 11.58), being grade level 1-4 (AOR = 3.91, 95% CI 1.69, 9.05), rarely taking a bath (AOR = 3.54, 95% CI 1.36, 9.25), contact with a person with itching symptom (AOR = 2.66, 95% CI 1. 21, 5.83), a family member with itchy symptoms (AOR = 4.76, 95% CI 2.20, 10.28), not living with both parents (AOR = 2.49, 95% CI 1.02, 6.06), and using water only for hand washing (AOR = 4.38, 95% CI 1.78, 10.76) were factors associated with scabies infestation among schoolchildren.@*CONCLUSION@#The prevalence of human scabies among schoolchildren in Dabat district northwest Ethiopia was high. The school localization, first cycle level of education, paternal educational status, frequency of taking a bath, and contact with a person having itchy lesions, presence of a family member with itchy lesion, and type of frequently used hand washing material were the factors significantly impacting the occurrence of scabies. Special attention should be given to students at first cycle education as they are at the highest risk of infestation.

ABSTRACT

Scabies is a common infestation worldwide, affecting persons of any age and socioeconomic status. In Singapore, it is more common in institutions rather than in homes. The two variants are classic scabies and crusted scabies, with the latter having a significantly higher mite burden. Early identification, isolation of index patients and prophylactic treatment of contacts are essential in dealing with the outbreak. Locally, most primary care practitioners make the diagnosis based on visual inspection and clinical examination. A skin scrape is done to confirm the diagnosis, especially in atypical presentations. Scabietic mites, eggs or faeces can be seen on microscopy. The usual treatment for adult scabies in Singapore is the use of topical malathion or permethrin. A combination of topical permethrin and oral ivermectin is used for crusted scabies.
